Co-Founder 

Dr. Jay Patel is a physician-entrepreneur and CEO dedicated to transforming serious illness care through evidence based practice, technology, data, and scalable clinical innovation. With nearly a decade and half of experience in palliative care, pain management, and healthcare leadership, Dr. Patel brings a unique blend of clinical insight and systems-level strategy to the innovations in health care industry.

He completed his internal medicine residency at The Brooklyn Hospital Center/Weill Cornell University and a fellowship in Hospice and Palliative Medicine at the University of Nevada, Reno. As the Founder and CEO of Pain and Palliative Associates, he has led the development of patient-centered, tech-integrated care models that improve outcomes for people with advanced illness across Arizona and currently leads the practice of more than 50 practitioners. 

Previously, Dr. Patel served as Division Chief of Palliative Care at Dignity Health, where he built cross-functional programs that aligned clinical excellence with operational sustainability. He also launched and directed a nationally recognized hospice and palliative medicine fellowship as Program Director at Creighton University School of Medicine.

A strong advocate for global access and equity, Dr. Patel is serving on the board of Pallium India USA and continues to contribute to thought leadership in health care innovation, medical education, and community health. He is active on multiple clinical governance committees including Ethics, Credentialing, Quality, and Graduate Medical Education.

When not building or advocating in the health care industry, Dr. Patel enjoys time with his wife and three children, as well as hiking, traveling, and exploring movies. 

Co-Founder

Patrick has over 20 years of experience in building and scaling successful SaaS business models with product offerings that have AI/ML at their core. Patrick is extremely passionate about solving complex market challenges, identifying niche market opportunities, and leveraging strategic channel partnerships especially in SaaS environments. Early in his career, Patrick specialized in corporate turnarounds and executive team restructuring projects for companies ranging in size from small family office operations to Fortune 500 members. Patrick has successfully led and built teams of over 250 employees across the United States and abroad. Moving to Scottsdale, Arizona in 2014 from New York City, Patrick Co-Founded REACH.ai, a global leader in artificial intelligence and communication technology, which solves complex scheduling challenges and automates communications between appointment-based businesses and their clientele. Patrick has experience leading capitol raises for organizations in early angel and seed rounds as well as securing 7 and 8 figure rounds for more mature organizations. Patrick holds an MBA with a concentration in Marketing as well as multiple post-secondary certifications in artificial intelligence and technology related studies. 

Born in Leiden, Holland and raised just outside of Boston, Massachusetts, Patrick was exposed early to a multi-national mindset instilled by his parents and extended family on both sides of the Atlantic. Patrick consults and advises multiple organizations, primarily in the United States and Europe, and is a serial entrepreneur.  When Patrick is not working, he and his family are avid travelers, and can always be found spending time with friends and family.

Frances is a RevOps Executive with a successful track record of driving business growth and brand development in diverse industries, including technology, media, education, and healthcare. Possessing a unique ability to work within extremely complex data sets to interpret operational deficiencies and gaps preventing optimal scaling and growth, Frances’ teams are well known for their world-class culture and for their ability to consistently outperform performance benchmarks. Frances has a true passion for training and developing junior talent while also leveraging her data analytics prowess to guide leaders under her charge. 

Most recently, Frances serves as the Vice President of Operations for Provide Media, a NY based portfolio conglomerate specialized in performance marketing and lead gen. During her tenure at Provide, Frances has played a pivotal role in optimizing operations and delivering innovative solutions through their multiple acquisitions, allowing Provide to increase their market share position in the highly competitive communications technology space.

Frances holds an Masters Degree in Adult Education and Training and is proud to call University of Miami her undergraduate home; Go Canes! When not working, Frances enjoys traveling with her husband and son, and spending time with friends and family. 

Co-Founder

Palak Patel is an accomplished healthcare and data analytics professional with a diverse academic and professional background. She holds a Bachelor’s degree in Biomedical Engineering from the University of California, Irvine, and a degree in Computational Mathematical Sciences from Arizona State University.

She began her career as an Actuarial Analyst, specializing in Medicare and Medicaid reserving, forecasting, and risk modeling, before transitioning into healthcare analytics as a Data Analyst for the HCAHPS team, where she focused on patient experience metrics and quality improvement initiatives.

Currently, Palak serves as the Managing Partner at Pain and Palliative Associates, where she leads non-clinical operations for a team of more than 50 employees. In this role, she oversees strategy, operations, and organizational growth—ensuring compassionate, efficient, and high-quality patient care. Her passion lies in building innovative solutions that enhance efficiency and keep healthcare truly patient-centered.

Outside of her professional work, Palak enjoys cooking, traveling to new places, and spending time with her husband and their three children. She brings the same curiosity, creativity, and dedication from her personal life into her professional endeavors.
